This form is only to be used for reporting damage, shortage or loss to goods despatched by us direct to the delivery address, or where the goods have been ordered directly from us and have failed to arrive at the intended destination. Strict time limits apply to all claim notifications so please make sure that all the information you give us is accurate and in full. Please also bear in mind that the carrier may be given a copy of this information to support your transit claim. Photographs may be required for all damage claims.
